At Holistic Evaluations Clinic, we provide medical marijuana certification to patients whose medical condition qualifies them for it. To get your medical marijuana certification, make sure you have your WA state driver’s license or ID before giving us a call to make an appointment where we’d inform you concerning the necessary documentation you’d need to bring along for your appointment. During your appointment, one of our qualified doctors would perform a non-invasive evaluation on you to determine if your medical condition qualifies you for a medical marijuana card. If you qualify, you will be given a 1 Page authorization form good for either 6 months or 1 year then you have the option to be entered into the WA DOH database.
