Holistic Evaluations is a trusted primary care and medical marijuana evaluation clinic at Renton Washington that was founded in 2011. Dr John Gillman, M.D. and staff of Holistic Evaluations strive at every step to make patients feel welcome and comfortable and to provide them with quality and compassionate healthcare. At Holistic Evaluations Clinic, we provide medical marijuana certification to patients whose medical condition qualifies them for it. To get your medical marijuana certification, make sure you have your WA state driver’s license or ID before giving us a call to make an appointment where we’d inform you concerning the necessary documentation you’d need to bring along for your appointment. During your appointment, one of our qualified doctors would perform a non-invasive evaluation on you to determine if your medical condition qualifies you for a medical marijuana card. If you qualify, you will be given a 1 Page authorization form good for either 6 months or 1 year then you have the option to be entered into the WA DOH database.

Q Certification Process Overview

Step 1: Connect with the Doctor/Clinic and request your certification appointment.

Step 2: Complete your certification appointment (in-person or online, you may be asked to submit medical records). Make sure that you (or the Doctor) have registered your profile with the State's Medical Marijuana Registry to submit your certification request with the State.

Step 3: You should receive your Medical Marijuana Certification within 1-2 weeks. Your physical card will arrive in the mail.
